Skim Insurans Kesejahteraan Rakyat (SIKR)

SIKR is a complimentary Shariah-compliant annual group term takaful plan, with contributions sponsored by the Government of Malaysia. The scheme is offered exclusively to heads of households from the hardcore poor and poor categories registered with the eKasih database under the purview of the Implementation Coordination Unit of the Prime Minister's Office (ICU JPM).

Exclusively managed by PruBSN Takaful Berhad (PruBSN), this one-year plan provides lump sum benefits in the event of death due to natural or accidental causes, or in the event of Total & Permanent Disability (TPD) due to any unforeseen circumstances.

How to Make a Nomination

Why is it important to make a nomination?

  • Nomination is important to ensure that your loved ones will receive the protection as intended by you and that PruBSN can pay certificate benefits out without delay. The nominee can be either an executor (Wasi) or a beneficiary under a conditional Hibah (Gift).

  • Without a nomination, the certificate benefits will be paid out to your lawful executor or administrator and distributed in accordance with the applicable distribution laws. This process may take a while.

  • It is essential to inform your nominee about the certificate benefits and nomination.


Who can I nominate for my certificate?

  • The nominee must be an individual. Nomination of an organisation is not allowed.

  • If your nominee is under 18 at the point of claim: the benefits will be paid to a surviving parent or proper claimant, i.e., legal guardian, subject to the relevant legal evidence/supporting documents.

How to make a nomination?

  • You can nominate your nominee(s) by completing the nomination form with a witness and submitting it via the following channels:

    1. Walk in at any PruBSN or Prudential branches; or
    2. E-mail to SIKR@prubsn.com.my.

  • Or any other channel PruBSN may make available to you from time to time.

  • The nomination form can be downloaded here, or obtained from

    1. Any PruBSN or Prudential branches; or
    2. The e-mail you receive upon issuance of your Certificate.

How to Submit Claims?

How to Submit Claims?

For a claim on Death and Total and Permanent Disability (TPD) Benefit:

  1. The claimant may submit the completed claim form(s) with supporting documents to any PruBSN branch.

What are the documents needed for claim?

For a claim on Death Benefit:

The claimant is required to submit the following:

  1. Death Claim Form - signed by the claimant (the person who is making a claim)

  1. Medical Attendance Certificate - signed by the doctor who last treated the deceased

  1. Certified True copy of Death Certificate - to be certified by PruBSN branch staff or the Executive/Business Development Manager

  1. A copy of Claimant's MyKad (if there is no nomination)

  1. A copy of Deceased's MyKad

  1. Certified True copy of proof of relationship (Marriage Certificate, Birth Certificate, Children of Claimant, Parent of Deceased) for all claimants (if there is no nomination)

  1. A Direct Credit Facility Form completed by the nominee/claimant

  1. Other supporting tests/Laboratory Reports: Biopsy, MRI, scan, and investigation results (where applicable)

All relevant claim forms can be downloaded here or obtained at any PruBSN branch. Claim forms can also be requested by sending an e-mail to SIKR@prubsn.com.my or by contacting the PruBSN Customer Service line at 03-27757188 (Monday to Friday, 8.30 a.m. - 5.15p.m.).

For a Total and Permanent Disability (TPD) Benefit claim:

The claimant is required to submit the following:

  1. Photo of MyKad (front and back)

  1. Selfie of claimant holding their MyKad (for verification)

  1. Copy of Bank Passbook / Bank Account Statement (front page)

  1. Total and Permanent Disability Confidential Medical Certificate (completed and signed by the doctor who treated the Covered Member)

  1. Other supporting reports - Latest physiotherapy/rehabilitation progress, doctor referral letters, blood tests, urine tests, biopsy, MRI, CT scan, ultrasound and all investigation reports (where applicable)

  1. Certified True copy of employment termination or medically boarded-out letter

PruBSN may request other information, relevant supporting documents, or request to view original documents whenever necessary.

How long does it take to process a claim?

The claim payment will be processed within 14 working days upon receive of complete documents.
